What is FollowMyHealth?
FollowMyHealth is an online tool that gives you anywhere, anytime access to your personal health records. This allows you to take a proactive role in managing your care. Many healthcare providers and physicians use FollowMyHealth as their main engagement platform. While your platform may have a different name, the technology is the same. FollowMyHealth is available online 24 hours a day, 7 days a week via any computer, tablet, or smartphone.
With FollowMyHealth You Can:
- Review your medical records online in a safe, secure environment
- Communicate privately with providers via secure messaging
- View test and lab results
- Read medical notes from your provider
- Update your health information (allergies, medications, conditions, etc.)
Portal Disclosure Statment:
The FollowMyHealth patient portal will not have any dental records or dental health history available. If you wish to obtain a copy of your dental records you may contact Open Door for a Release of Information (ROI) form that will initiate the process. Open Door Health Center has 30 days to release the records once the ROI is completed.
