Self-Attestation Option

90-day discount available without documentation

New patients who cannot provide documentation may self-attest one time for a 90-day discount. Patients with no income may self-attest every 90 days.

Insurance

Most private insurance plans are accepted, as is Minnesota Care and Medical Assistance (MA). If you have insurance from one of the providers listed below, your services are likely covered. Please discuss with your insurance company whether we are an ‘in-network’ provider for your plan.
