Sliding Fee Discount Program
For those who do not have insurance or have high deductible insurance, we offer a sliding fee discount program based on several factors,
including income and household size.
We offer five levels of discounts to those who qualify. We do not quote this information over the phone but request a face-to-face interview to discuss your economic situation in more detail to best help you. When you come in to meet with one of our Patient Service Representatives, we ask that you bring with you one of the following for proof of income: W-2’s or pay stubs from the last month, your most recent Federal tax return, or social security or other public funding letters.
Please complete this form if you cannot provide documentation of your household income. NEW Patients (never seen at ODHC before) can self-attest one time regarding their household income for a 90-day discount. Patients with NO INCOME can self-attest every 90 day.

Right to Receive a Good Faith Estimate
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of their bill for health care items and services before those items or services are provided. Read the full document.
Insurance
Most private insurance plans are accepted, as is Minnesota Care and Medical Assistance (MA). If you have insurance from one of the providers listed below, your services are likely covered. Please discuss with your insurance company whether we are an ‘in-network’ provider for your plan.
