Open Door Health Center is seeking a qualified candidate for a full-time position as a Behavioral Health Consultant for our Medical and Dental patients.

Job Responsibilities:

  • Provide assessment, consultation and intervention for patients, providers and staff in the area of psychosocial factors on a same day and/or scheduled basis.
  • Psychosocial and diagnostic assessments, treatment planning, internal and external referrals, crisis assessment and intervention
  • Maintain a visible presence to the dental providers and dental staff during clinic operating hours.
  • Teach patients, families and staff prevention and treatment enhancement techniques.
  • Assist the provider and care staff in developing care management processes to include patient education to improve self-management of chronic disease.
  • Responsible for continuous assessment of needs presented by ODHC patients and staff and by the community as they relate to patient advocacy and providing quality patient care and services.
  • Ensure the provision and acquisition of economic, social, and emotional support services to ODHC patients through integrated primary care.

Minimum Qualifications

  • Master’s Degree and MN state licensure as a Behavioral Health Provider (LGSW or LICSW) is a must.
  • A minimum of 2 years related work experience providing mental health services.
  • Abide by ethical standards of licensing board.
  • Comply with credentialing requirements.
  • Strong interpersonal skills.
  • Exceptional verbal and written communication skills.
  • Excellent time management and organizational skills.
  • Strong interpersonal skills.
  • Successful crisis management skills with experience in diffusing difficult situations.
  • Have a basic understanding of medical diagnosis and appropriate behavioral health interventions.
  • Confidence, professional judgement and grace under pressure.
  • Bilingual skills a plus.
