In Minnesota, all athletes in grades 7-12 must have a sports physical to participate in school sports.
Sports physicals are valid for three years and are crucial in helping your teen play safely and prevent injuries.

Sports Physicals FAQ
Open Door Health Center offers sports physicals for $20.
Anyone, including existing or new patients, can schedule a sports physical with Open Door Health Center. Sports physicals must be scheduled by a parent or guardian, and they must accompany their teen to the appointment.
Sports physicals at Open Door Health Center include a medical history review and a physical examination. Once your teen has been cleared for sports participation, you will receive a completed Minnesota State High School League sports eligibility form to bring to your school. If your teen is not cleared for sports participation, we will work with you to reevaluate your options and next steps. Sports physicals are valid for three years in Minnesota.

For your sports physical at Open Door Health Center, please bring a copy of your teen’s immunization records, medical history, list of medications, and any medical devices your teen may use, including glasses, contact lenses, hearing aids, etc.
