Community Health Worker - Mobile
Open Door Health Center (ODHC) has an opening for Community Health Workers (CHW). The CHW is responsible for reducing barriers to health and human services among vulnerable populations. He/she works to ensure integration of care within the context of the primary medical, dental and behavioral health departments, and promotes wellness by providing culturally appropriate health information to clients, providers and families. The CHW provides health care literacy through advocacy with the individual, the family, community and other health care organizations to ensure the community capacity meets the required needs of all individuals.
The CHW must have a certification in Community Health or a degree in Social Work or related field, along with 1-2 years’ experience in case management, community health, counseling, social work or other related field. Suitable experience could be exchanged for education. He/she should be knowledgeable of community organizations that can be of assistance in meeting the needs of the patient, as determined by the Community Health Coordinator. Verbal and written communication skills are required; bilingual language skills are a must. The CHW must have the proven ability to manage multiple or competing priorities, work with a diverse population of people, be detail oriented, and exhibit excellent follow up skills. He/she must also have the ability to work independently with minimal supervision and in a team oriented environment. Bilingual skills are a requirement.
Travel Requirements / Work Hours
Travel is required on a regular schedule, to attend meetings, events and to provide support within the community and across the region using a mobile unit, rental vehicle, or even personal means of transportation. Must possess a valid driver’s license and have a good driving record.
