Position Summary

Open Door Health Center (ODHC), a federally qualified health center, is seeking a full time Behavioral Health Consultant.

The position of Behavioral Health Consultant (BHC) encompasses many facets of job duties; those duties will be determined by Open Door Health Center, the departmental needs, and individual abilities, experience and/or education. This position will be under the direct supervision of the Director of Behavioral Health for patients/clients served by ODHC’s model of integrated care.

The primary responsibility for this position is to have the ability to provide comprehensive assessment and diagnosis of medical, dental and behavioral health clients, and effective treatment planning and assisting clients in successfully achieving goals. Evaluate crisis situations and apply appropriate interventions. Provide short-term intervention and/or supportive counseling. In addition, assist in the detection of “at risk” patients and development of plans to prevent further psychological or physical deterioration. Provide evaluation, assessment, and intervention for patients exhibiting social and emotional difficulties affecting their health status, and develop, provide, and systematically re-evaluate the plan of treatment for patients/patients families. Make referrals for patients when appropriate services are not within the ODHC Behavioral Health Department scope of practice and assist the primary care team in developing care management processes such as the use of guidelines, disease management techniques, case management, and patient education to improve self-management of chronic disease. Provide assessment, consultation, and brief intervention for psychological/psychiatric problems and/or disorders. Provide education for patients, families, and staff care, prevention, and treatment enhancement techniques and consultation for patients in the area of psychosocial factors which might affect adherence to treatment, symptom management, health promoting behaviors, health related risk taking behaviors, and overall adjustment to medical illness or chronic disease management.

Minimum Qualifications

The Behavioral Health Consultant will have a Master’s Degree in Social Work and a minimum of two to five years’ experience providing mental health services. Must have strong interpersonal skills and be able to communicate effectively with all levels of the organization, and with patients, visitors, and outside agencies. He/she should demonstrate strong organizational skills, the ability to prioritize and handle multiple demands, and the ability to develop, lead and facilitate staff to achieve the goals of the organization.  He/should demonstrate successful crisis management skills, with experience in diffusing difficult situations.  Training experience is preferred. Experience with a Patient Centered Medical Home is a plus.  Candidates with interest and experience working with adolescents, families and/or groups are encouraged to apply. Experience working in a community health setting with diverse populations and bi-lingual (English/Spanish) skills are a plus. Ability to fluently speak, read, and write the Spanish or Somali language and has a good understanding of the Latino and/or Somali culture, preferred. Demonstrate strong interpersonal skills, the ability to communicate effectively with patients/clients, clinical providers, support staff and have strong organizational and conflict resolution skills and attention to detail. Demonstrate the ability to set goals, prioritize work load and work in a fast-paced and sometime stressful environment.
